World Record for Plastic Surgery?

"Angela Bismarchi, 36, is hoping to set the world record for most plastic surgeries. The Brazilian model has had 42 operations, making her a celebrity in her home country."

This one is kinda odd, but she seems happy about it. It is both fortunate (practical) and unfortunate (financial) that my practice doesn't appeal to this kind of patient.

LOL. I probably wouldn't do these cases anyway.

A Silicone Gel Breast Implant Story on AbsolutelySafe.com

This piece features the story of a woman who has had problems with a breast reconstruction that involved silicone gel breast implants. It also makes the supposition that all of us in plastic surgery have unanimously proclaimed that silicone gel implants are "safe." The piece fails to differentiate between cosmetic breast surgery patients and reconstructive breast implant patients. They differ substantially in the risk of complications.

These people obviously haven't read anything I have written.

John Ritter's Death Lawsuit - Benefit versus Risk

Clinical medicine is a minefield. Looked as "Benefit vs Risk" it is very much in negative territory. This is why so many doctors trained in other fields leave them to practice cosmetic surgery. While I do not know the Ritter case, it is safe to say that the millions sought after by the plaintiff vastly dwarf any money paid for the care of Mr Ritter. While I still do some regular medicine, I am actively suppressing that portion of my practice. It simply isn't worth the risk.

Porn Stars & The Rare Full Breast Lift

Years ago I wrote a piece on my web site about Porn Stars and their Misadventures with Cosmetic Breast Surgery. A friend provided images from the web including some of a lady who went by the name "Anita Dark." She was #3 on the page.

In surfing today I found more recent images of Anita at a softcore site Bullz-eye taken in 2004. She is one lovely lady making me wonder why she ended up in porn. Nevertheless, she has had at some point a full breast lift or breast reduction with a complement of incisions that appear to have healed pretty well.

My point is that patients who have droopy breasts should consider breast lift surgery and not just placement of large breast implants. I see far more problems with large breast implants than with breast lifts (when properly performed of course). There are of course less invasive types of breast lifts for women with less droopy breasts or those willing to trade less correction for fewer incisions.

Dr Phil Gets slapped on the Britney Spears Visit?

It looks like the Spears family is trying to get back at Dr Phil on the Britney Spears hospital visit. They have filed a complaint with the California Board of Psychology and apparently also filed to sue him.

The problem: Mr McGraw no longer practices psychology. He relinquished his Texas license in 2006 per TMZ and has never had one in California. When someone gives advice of the sort he gives, does that define psychology? If so, every friend to ever give advice to another could also be sued. He is an entertainer now. I think they are barking up the wrong tree. Maybe he should have kept his mouth shut, but when you call an entertainer what do you expect?

Wii problems and Costco

The kids got a Wii from my Dad this Xmas. It was great for 2 weeks and then we had a remote problem. Costco is usually a great place to buy stuff, but really supports the Wii poorly. They told us to return it as they would not reserve an incoming unit for us (as an exchange) eventhough it had been purchased there receipt or not. Although I am usually a huge Costco fan, I would recommend buying your Wii elsewhere.

Interestingly enough, a call to Nintendo yielded a request that I smack the remote around to make it work again.

Transgendered Patient Sues Hospital

Trying to figure out why a patient would want to push a surgeon into doing an operation he/she wasn't happy to offer has always confused me. The surgeon / hospital can't win here. If the procedure doesn't work out to the patient's satisfaction, then the lawsuit becomes an issue of poor performance.





If I am not comfortable, I do not operate. Believe me, unless I am comfortable you really don't want me to work on you.

Never having done breast augmentation upon a "previously" male chest, I would not know where to start and could not make any kind of assurances on the outcome. Would a patient really want that? In this case, the hospital refused on religious reasons. Although I believe they should have the right to do so, they are probably screwed.

The lift surgery "takes up the slack" of the excess skin we get when removing larger implants that have been in a while and either replacing them with smaller ones or not replacing them at all.

Donda West Health History / Cosmetic Surgery Candidacy

Donda West had high blood pressure and "high blood sugar" (probably diabetes) according to the LA Times.

I find myself agreeing with Dr Rohrich as he states:

"It's not like going to find a pair of shoes. You can take the shoes back. You can't bring your life back."

"Bottom line, cosmetic surgery is only for patients that are healthy. If they're not healthy, it's really not usually a good thing to do."

The real question in the review of this case will likely be whether or not Ms West was a reasonable candidate for the surgery in the first place. The subject of medical clearance will undoubtedly also come into play.

Donda West died of a Heart Attack; had a Lower Body Lift

TMZ

TMZ has posted a copy of Donda West's coroner's report (How did they get this?) that reports that she died of a heart attack and various post-operative factors. It also reports that she had a circumferential tummy tuck or a lower body lift. This is large scale surgery.

One of the reasons a surgeon may seek medical clearance from an internist is to minimize the medical risk of large scale surgery particularly in a person not in the best of health. It is my understanding that she didn't have this done prior to surgery.

This isn't over if the medical board decides to investigate. I am not placing blame upon anyone. This case is not open and shut however.

BTW - This was not a breast reduction technically. It was am implant exchange/lift.

Victoria Beckham's Breast Implants - Again?

HollywoodTuna has a post today featuring Victoria Beckham's breasts looking kinda freakish as they did a few months ago in most of her pictures. There were images of her looking a bit less firm upon which I commented in late 2007.

What gives?

Looking at what Mrs Beckham tends to wear (such as in her latest pictures,) she can make even the softest implanted breasts look firm. It is hard to tell with this kind of clothing.

I'd still lay my bet upon her having had them re-done late last year but there is not much that is certain when you examine images of clothed celebrities alone. That is what makes this so interesting, yes?

It is also possible that her new implants have started to "firm up." Contracture commonly recurs particularly in women who smoke and women who had silicone gel implants. I do not know the specifics in her case.

"Real Housewives," The OC Register and "My Breasts are too big with these Implants!"

This week the "Real Housewives of the OC" featured a woman who had old breast implants reduced and her breasts lifted. A number of plastic surgeons were asked to provide commentary. I did so but my comments were not used. Nevertheless, the issue is worth discussion as this event happens in reality more commonly when women choose to "go too large."

My Commentary:


Breast implantation results undergo an evolution of sorts as patients age. With the weight gain many patients experience as they age, breast size increases and the larger breasts may descend. Breast implants of course just add to this weight. With the movement of increased breast weight to a lower position on the chest, sometimes back pain can occur usually along the course of ten to twenty years after surgery. This is not universal, but it happens.

Because of these changes, there are not infrequently patients that come to my office considering removal of breast implants frequently along with breast lift surgery. I do a few such cases a year in which I remove implants for weight concerns. If there is enough breast tissue remaining to produce attractive breasts, I make an attempt not to replace the implants. Some patient will want the implants back.

A point of education would be to consider smaller goal sizes in "first time" implant surgery with the idea of decreasing the possibility of something like this happening. As the leading cause of dissatisfaction in augmentation is "too small" a size result, this produces a paradox that patients should carefully consider before surgery.
