A "Dr Quickie" Story


- THIS IS NOT A DR DISAIA PATIENT -


As many of you know, I contribute online at a few forum boards answering some general plastic surgery questions. At a MakemeHeal.com thread today, I ran into a post by the husband of an extreme weight loss patient who had had a tummy tuck and a breast reduction by another surgeon both in 2 1/2 hours. They are both unhappy with how she looks. They did not post pre-op images. A six week post-op is above.

As we have discussed before, extreme weight loss plastic surgery is more extensive than similar operations on a normal patient. 2 1/2 hours is very rapid for surgery of this type. I can safely say I would take much longer on each of these procedures and would likely do them in two different operations.

Another post in the thread indicated the surgeon's name. I searched it. He is not board-certified by any ABMS board (American Board of Medical Specialists.)

Just looking at the surface, it is really irregular and the closure was with staples. The surgeon appears to not have re-implanted the belly button and a good deal of looseness of the tissues still exists above the closure. I hope she didn't pay much for this.

Lipodissolve News Video - Be Careful.

Mesotherapy is a popular request these days. The idea that you can melt away fat without liposuction is attractive. Mesotherapy works by injecting a "fat dissolver" under the skin (subcutaneous injection.) The problem is that these therapies are unstudied although they have been offered in Europe for years. In the video clip linked above, Lipodissolve is featured in a Minnesota news story. They don't tell you that the material injected has a warning against it in Great Britain (by the Medicines and Healthcare products Regulatory Agency (MHRA)) and the manufacturer does not recommend injection under the skin (subcutaneous injection.) This is the technique used in these cases.

These therapies need to be studied. I do not offer them and would not recommend them.

Coco's a cartoon?

When I see images of Coco (the gal with Ice-T,) a reprisal of an old post comes to mind:

Implants of the size we see here dwarf the tissue [she] has available for coverage.

When you go really large on breast implants, they are going to look fake. As long as that is what you want, I guess that is OK. They probably hurt too at least on occasion at this size. Smaller ones really don't.


We were talking about Pamela Anderson, but we might as well be speaking of Coco. The same problem is in motion. It is just earlier in the evolution.

Plastic Surgeon Testifies in Tom Sizemore/Heidi Fleiss Case

A plastic surgeon testified that he didn't see bruises on the face of Tom Sizemore's former girlfriend, former Hollywood madam Heidi Fleiss, days after she claimed he punched her in the jaw at a Beverly Hills hotel.

"There was no sign of any trauma, bruising or anything like that?" Fay Arfa, Sizemore's attorney, asked Dr. Michael Cardenas on Friday.

"I did not notice anything like that," said Cardenas, who said he had a conversation with Sizemore and Fleiss around 2 a.m. April 13, 2003, about five days after the assault.



This is an easy case to refute. Signs of bruising do not always appear after blunt trauma.

Blogger Question - Victoria Beckham's Leg



The Question:

This is a link to a picture of Victoria Beckham's leg (http://img.dailymail.co.uk/i/pix /2007/07_02/beckham RTN1907_600x807.jpg) and I was wondering what happened with her skin. Would sun damage do that?


An Answer:

It is likely that she had gained and lost weight over the years resulting in loose skin that is receiving direct sunlight and therefore is affected by unflattering shadowing. The weight gain could have been with pregnancy and the tendency for her skin to become loose could be genetic. The sun is only indirectly responsible for this one.

Jessica Simpson Back On Lip Fillers

Jessica Simpson here looks like she has had a fresh batch of lip filler like Collagen, Restylane or Captique in her upper lip. Either she had too much filler or too much local anesthesia (for my tastes anyway). I like less filler, but I'll admit I am in the minority.

I remember her former statement that she didn't like the lip filler experience. Maybe she changed her mind?

This story is old but the bottom line rings true today: Choosing surgeons based upon price alone is a loser.

Slim Training Before and Afters

I just watched this Slim Training cable television ad. Looking at some of the "Before" and "After" images, they appeared as if they had had surgery. One had an appendectomy scar that disappeared with her weight loss. That doesn't happen without surgery folks. Several had belly button changes consistent with tummy tuck surgery.

Be careful with this one.

Tummy Tuck Wound Care - Ask Your Surgeon.

I occasionally frequent an online forum or two in which people get together to discuss issues relevant to their procedures. I have no problem with this obviously or I would not participate. Recent postings include:

While online discussions are fine, in new post-operative patients I wonder why these patients aren't asking these questions of their surgeons. I think post-operative patients should be getting wound care advice from their surgeons. I would not want one of my surgical patients changing the management of a wound or a healing scar without my knowledge. This is part of what they pay for when they employ me to do their surgery.

I think I see my cosmetic patients more than other surgeons after the surgery.

Lisa Rinna - Good Shape Plus Skin

Exercise can help with fat but skin will not shrink. Here's an image of Lisa Rinna's tummy from the side in what appears to be good shape (more pics at Yeeeah.com link above) but her skin from pregnancy remains. Seeing as she seems to be pro-Plastic surgery (Lisa's +++++ lip filler images abound online,) it seems funny that she waits on her tummy.

Teri Hatcher - Too Much Botox?

Teri Hatcher can smile, but her forehead doesn't move much. This may likely be a sign of a number of Botox injections decreasing her forehead movements. This is one of the reasons I do not do whole foreheads with paralytics like Botox. They end up looking kind of funny. The preservation of some movement is desirable.

Tom Jones' Eyelids

TMZ has a post on entertainer Tom Jones with a recent image. He has had his share of plastic surgery, but looks like he needs some consideration for his left lower eyelid. It is a bit droopy. This could of course be due to previous surgery or just his age.

Jessica Simpson Lipo?

Dlisted blogs on Jessica Simpson with some close-up images of one leg. They ask if she's had liposuction.

Well she does have some irregularity up top near the side of her rear end. She might have had some liposuction, but I doubt her procedure was very recent unless she had one of those silly mesotherapy sessions. These cause less trauma but can lead to some irregularity.

I figure "Likely" on lipo, but not recently.
