Sue Johanson on Labiaplasty



I just saw this video on labiaplasty from Sue Johanson cable show. Ms Johanson, like many in the media, seems to assume all surgeons do them the way gynecologists have done them for many years by the "trim method." The "trim method" does reduce sensation. That is why I will not do them using that method. The various versions of modified wedge reductions do not reduce sensation. My patients are invariably pleased with their reductions. I follow them until they are back "in action."

Ms Johanson also fails to address the major reason women consider this operation: discomfort in clothing. When Labia minorae get really large, they hurt in underwear and can give the appearance of a "package" in gym clothing. I agree with the inference made in the clip that women should only consider this operation for internal reasons. Then again no partner of any of my patients to date has been the reason for her considering this surgery.

I usually like the excerpts I have seen from Ms Johanson's show. This one is just off the mark. Maybe it is from an old segment.

Celebrity Boob Jobs gone Wrong?

CityRag blogs on celebrity boob jobs. It is sometimes difficult to figure what goes awry with a breast implant case.

Some "educated guesses" based upon the images in this article however are:

* Janet Jackson - Breasts and implants droop over time. She may need a lift.

* Victoria Beckham - Probably old silicone gel implants "Over the Muscle." They contract quite a bit. Probably needs a redo with "Under the Muscle" implants. She doesn't seem to have much tissue for coverage, so she is best advised to stay small.

* Tori Spelling - Went too large and now after childbirth she is larger and "droopier." A lift and down-sizing might be in order.

The fourth woman in the article has images of an early result from which I can tell little.

Hilary Duff has Nice Abs and Liposuction is NOT the Reason

Derek Hail has a post on Hilary Duff's Abs. She is in good shape and likely got there the old fashioned way - diet and exercise. Liposuction can thin someone but it does not tone a muscle or tighten skin. Lipo alone will not make a six pack abdomen.

Liv Tyler Up Front On Plastic Surgery

Liv Tyler recently revealed to Allure magazine that she thinks she will go under the knife at some point in her future.

Liv, 29, told Allure, "I'm definitely going to have some [plastic surgery], I'm sure. Especially when you see what happens to your body after you have a baby." She connects her decision with work, stating, "I have to make a living. I guess I could sell my house and go live a really simple life in a trailer somewhere."


Post-pregnancy plastic surgery probably means tummy tuck plus or minus liposuction.

Liposuction is Not Weight Loss

I must agree with Dr Nahai here. This young girl should not have had liposuction. The real answer (if we get follow-up) will be whether or not she maintains her weight. Most liposuction patients do not maintain weight losses unless they change their lifestyle. It doesn't sound from this story like this girl did so.

"I almost died from (liposuction). I had it done all over and it didn't work at all. I realized, 'Oh, to change your body you have to eat less and go on a treadmill.' So that liposuction landed me in the hospital.


Comic Kathy Griffin reports on her experience with liposuction. She must have had it for weight loss because in my patients I have no such stories. She did discover the reality of weight loss however. She just discovered it the hard way.

The Bottom Line:

I perform liposuction only for patients who report that their weight is stable and they are not seeking weight loss. It is a contouring procedure. When performed in such a way it is not dangerous and it works.

Pre-Op Education - Porn Star Boob Job Example







We have discussed "all implant" boob jobs before. Breast implants can only help make breasts look good (in my opinion of course) when there is adequate tissue to cover them. When the size of the implant gets too large, we get distortion and a really "fake" look. We also get the increased possibility for re-operation for pain and implant displacement. In this case, bigger is not always better.

This image features two women in whom the implants are so large that they fall into "Will Probably Need More Surgery" club. The one of the left probably could have used a lift, but doesn't look anywhere as bad as the one on the right. She just went huge. I'd wager that both gals will be back to the operating room several times over their lives and may still end up looking horrid.

This is not necessary: Smaller implants and the use of a lift when needed can really avert all kinds of trouble.

Morgan Fairchild - Too Much Plastic Surgery?

As many of you know, I follow some celebrity blogs to obtain information for mine. One of my favorites is Truth, Beauty, Love, and Elisa because Elisita exudes sarcasm so thick it requires a knife to cut.

Today she dishes out some of that delicacy for Morgan Fairchild. Morgan who has seen her share of plastic surgery, looks pretty good for 60 (I take it that Elisa did her homework.) Then again the tendency is to compare celebrity plastic surgery "After" images to images of the aforementioned in their prime many years earlier. This is not really fair (no pun intended here.) The real question (really impossible to answer) is what might she have looked like without the plastic surgery.

Now that I have been fair, does anyone know if she was a smoker? Has her weight fluctuated quite a bit? These issues would make her fairly hard to operate and produce the best results. With this being said, she has probably had a bit too much surgery over the years.

Sometimes if you cough or sneeze a bit of wee comes out! I just have to cross my legs and hope it doesn't trickle down my leg.

Well, leaking urine when coughing is called stress incontinence. My father fixes those. He is a Gynecological Oncologist. She is really not looking for vaginal surgery at all. She is looking to fix her bladder. This has nothing to do with cosmetic reduction labiaplasty. I do those.

Blogs can seem cruel. I am sure they intend to be good humored about it. Victoria Beckham is an unusual personality by her media persona, but she doesn't really seem to have an acne problem. Oil maybe, but acne - not in this image. She also has a few pigment changes that a peel or two might help.

Awful Plastic Surgery has a bit on Victoria Beckham's boob job. Needless to say they are not very complimentary. I have seen far worse results. Mrs Beckham's result is pretty typical when a relatively large implant is used over little soft tissue cover. They just look really fake. Then again some women want that...for reasons I cannot fathom.

The Skinny On The Star's Cellulite

The Skinny posted on the Star magazine's "Cellulite Secrets" article including some Scans with pics from the Star.

Let's "Flesh Out" the piece a bit:

(1) Britney Spears - We have discussed her before. Her recent exposures (pun intended) have shown us all the "looseness" of her skin following weight loss. She has some fat left, but her major problem is skin excess/laxity. "Lipodissolve" even if it did work (which I am not sure it does) should have little effect on her remaining problem. She has improved a great deal from months past however it is difficult to figure what portion of her activities (diet, exercise, etc) may be responsible.

(2) Jessica Simpson - It is impossible to compare the included "Before" and "After" images as her position in each is substantially different from the other (one has her leg extended and the other has it in a neutral position). I am not sure there is a problem here.

(3) Beyonce' Knowles - same as (2)

(4) Uma Thurman - same as (2) and (3)

(5) Tara Reid - similar to Britney Spears except she had liposuction (twice) with weight variation before and after. Her skin is lax over some of the areas in which fat has been removed. Liposuction works best upon those in a stable weight range. Don't lose or gain a bunch of weight (more than say 7 to 10 pounds) after liposuction if you want the results to continue to look their best.

(6) Pam Anderson - She keeps herself in shape and has never really gained a lot of weight in her lower body. Her legs look pretty good especially for her age.


The Bottom Line:
The removal of fat can reduce the appearance of cellulite to an extent, but to have a greater impact the skin problem must be addressed. This is body lift territory folks....tummy tuck, thigh lift, etc. Of course avoiding the extent of weight gain in the first place can help prevent it. Genetics play a role here as well as does overall physical condition.

The appearance of cellulite is due to skin laxity as well as fatty deposits. Any modality that only affects fat will only minimally improve (if at all) the situation.

Apparently Hugh Hefner is looking into another facelift having had (as he indicated) a neck lift a few years ago. It should really be no surprise that he is pro-plastic surgery.

Pamela Anderson Doesn't See Her Bust As Large

The former 'Baywatch' star insists she doesn't recognise her surgically-enhanced boobs on screen because they look so much bigger.

She said: "I think my breasts expand on TV. I swear I don't feel that big. When I look at them on TV, I think, 'Who is that?' It's all lighting."

Britney Spears and Cottage Cheese

Britney Spears keeps getting her privates photographed. The Skinny posts about her "cottage cheese." This brings up a common misconception about the changes we see after weight loss.

The loose jiggly tissue around her rear end is not where fat is. It is really where more fat was. Just removing more fat here will not fix the problem.

When Britney lost her "baby weight," her skin became loosened more in the areas where the fat shrank from her weight loss. This gives you the "jiggly" fat we see in that g-string photo.

To make things tighter skin must be removed. Britney is becoming a lower body lift candidate.

I take a lot of heat from other plastic surgeons for the perception of a "cup half full" position in many of my posts. There are good cosmetic surgery results. The vast majority of mine are good or better. Then again I am qualified and take the time to do a good job. I also do not try to get a procedure to do more than that for which it was designed. This is called "playing it safe." It leads to far fewer problems.

Nevertheless, here is a patient of mine upon whom I performed liposuction a few years ago. More are on my site for those interested.

Redo Boob Job Of The Month



Here we have images of a woman I first saw a few years ago. She had had two breast implant operations before and noted after her second (an enlargement with full lift) that her left breast progressively dropped (over about a year). I recommended re-operation to re-set the lift and downsize her implants. She agreed to the lift but not the implant size decrease. Smaller implants do not weigh as much so the rate of recurrent sag should have been less. As these were silicone gel implants this is particularly important. They weigh more than saline-filled implants.

The After image was taken at 1 month. She moved Out of State, but I predict that the sag probably recurred at least to some extent.


The Lesson:

(1) Large implants are hard to keep up. Plan on smaller implants or more surgery later if you select larger implants. Large silicone implants also harden (capsular contracture) more frequently/severely than smaller implants or saline implants. This also contributes to a higher redo rate.
