Kirsten Dunst refuses to "fix" her teeth

"Messed-up teeth are so sexy."

Sounds like we have a junior Robert Redford here. He eventually got a facelift after swearing that it was not for him. Entertainers eventually feel pressure to consider surgery. No one by definition needs it. This is cosmetic after all.

We can follow Kirsten and see how her career and teeth fare. I'll bet she changes her tune eventually; that is, unless she gets away from the cameras.

Sharon Osbourne's DD Breast Augmentation

"I am very lucky in that I have a good cosmetic surgeon. The last thing I had done were my breasts about three months ago. I am a DD now and Ozzy loves them."

Reports are that Sharon has spent some $700,000 on cosmetic work. Is it too much? Gastric Bypass patients tend to get alot of body work. Most do not inflate their chest to DD proportions though. Breasts usually get quite droopy when inflated to this size in this type of patient. I would predict more surgery in the future on her breasts if all is as described.

She had a gastric bypass in 1999. Still, she is well over the usual price tag for post-gastric bypass work. She is a plastic surgery blogger's "frequent flyer."

Extreme Makeover - What Part The Makeup?




A friend with a gift for producing more e-mail than your friendly neighborhood surgeon can read at times e-mailed me a few pics including this one. It spurred a thought:



"What part of Extreme Makeover results are the makeup?"

This lady apparently had just makeup. Well maybe in addition to more flattering camera-work for the after image.

Having been a photographer in years gone by, I make a concerted attempt to make my before and after images honest. Not all surgeons agree. Many have patients come for their before pictures without makeup and take their after images with soft studio lighting and makeup. This is cheating. Any photographer should be able to use camera tricks (and now Photoshop) to make their results look better than they are. You are not at your surgeon's office to see how good the photography is, are you?

The bottom line is that you should look carefully at the pictures at your surgeon's office before you have surgery.

A Fixin' Your Thighs - Inner Thigh Lift

After reading a post at a forum that I occasionally visit, I figured an explanation of what thigh lifts can (and cannot) do was appropriate. Many plastic surgeons look at thigh lifts with a degree of exasperation. They not infrequently require revision. Those in large weight loss or gastric bypass patients can be a big job.

The amount of excess skin that so unattractively giggles in the inner thigh is the question. To reduce said giggling requires incisions. If the excess is mild to moderate, we can usually get away with just an incision in the crease between the thigh and groin. Gastric bypass and extreme weight loss patients often require an additional incision vertically down the leg. These incisions heal with variable scarring. The scars can migrate down the leg although this is less frequent than it was years ago.

We pull and often liposuction to make the tissues as tight as we can. Then to an extent they loosen over a year or so then we see how we did.

"Vaginal Rejuvenation" Again in the News

"News of the Wierd" MSN Piece

Again there is a news report (this time in MSN's "News of the Wierd") mentioning genital surgery. As usual the discussion centers upon the unusual at least relative to my practice. To be fair, the article is entitled "News of the Wierd."

My own experience relates to outer vaginal lip reductions and not "tightening." I am a plastic surgeon, not a gynecologist. I have yet, however to have a single patient indicate to me that her reason for having the surgery related to a partner. The ladies have all desired that their Labiae be reduced for their own reasons usually embarrassment and/or discomfort in tight clothing. Similarly, not a single patient has indicated that she wished her genitals to look like those of a porn star.

So the article is somewhat misleading relative to the routine case. I guess this makes for better reading.

Weight Gain After Tummy Tuck Question

I visit forums other than my own less frequently now than in years gone by. This question is a common one concerning fat returning after a tummy tuck:


The Post:

Almost a year ago I had a FTT that had to have revisions 14 weeks after but>>>>>>Now I am growing these square "pads" of fat just above my butt cheeks on my lower back!!!!


My Reply:


This is the reason I emphasize the stability of weight before a tummy tuck or liposuction. People that gain and lose more than a little weight (3-5 lbs) frequently are the ones that have this "migrating fat" phenomenon after large scale body contouring surgery. They are also the ones that have little if any improvement after small scale liposuction. I tell people this on the "front end" of course.



A Tummy Tuck will not help you lose weight or assist you in keeping it off. It is a contouring procedure only.

Jordan to tame "Monster" Breasts

Jordan, the UK's FF cup page 3 girl, is going to have her implants reduced...probably downsized. Anybody know how large she was before implants? This could be challenging surgery. How much skin she recruits by downsizing will determine how much surgery she needs. If she was really flat before implants, there may be quite alot of skin. Larger implants always descend with time and childbirth. That is a reason why I don't place celestial bodies. I have a feeling her implants are pretty huge compared to her frame.

Mesotherapy..a slightly biased perspective

Someone asked me about mesotherapy online today and a patient in the office this week asked as well. Plastic surgeons really do not offer this alternative treatment. I only have the reports of a few of my patients that have had it:

One believes that it has been helpful after her liposuction.

One believes that it left her "bumpy" and needed an extension of her abdominoplasty with some liposuction to help fix this.

Mesotherapy involves the injection of a number of things into your fatty tissues to make them shrink. The idea is to make them shrink uniformly. This may not always happen. There is no uniformly accepted formula as to what should be injected. There is no oversight of which I am aware to make the experience safe. There is no specific training for practitioners who provide it. This is uncharted territory.

For those considering mesotherapy, I would consider it as an experiment. You don't know how it may work for you until you try it. There are no good studies of which I know to tell you how it may work for you.

Medical Advice - "No" to Liposuction

Today in the office, I saw a nice gentleman of nearly 50 years who wanted liposuction of his upper and lower buttocks. What he has there is skin. Liposuction is not right to address skin. He is not at all fat. I told him the truth and he left without a recommendation for surgery.

Now and again I see a patient for whom I similarly recommended no surgery that returns after having it somewhere else. Sometimes the outcomes on what I considered ill-advised operations are scary. They go to validate my previous position, but don't make me feel better.

When a surgeon that makes his living operating recommends no operation, I think that should be carefully considered advice.

Legal Foolishness- Sued for Not Telling You You Are Fat

Now a lawyer is warning of obesity suits. He is telling medical students that they need to warn their patients of the risks of their obesity. Not long ago we spoke of a doctor being sued for the way he told a patient she was obese.

Is the bottom line no matter what you do, as a doctor you will be sued?

Maybe as a public service we need to write a legal disclaimer for doctors-

Dear potential patient,

At the advice of counsel, we have been urged to let you know that you are fat. Being fat can make you prematurely die. Please do not be offended in any way. This is a public service message.

News - Doctors linked to medical device companies

CNN Story

I only wish. In other areas of business, it is OK for a person to be involved in a business relationship with an allied product or company. For some reason in health care, it is forbidden. While I see the point if the product is inferior and being used because of the financial bias, most often this is not the case.

Doctors used to own laboratories and make money from that relationship. With that came the responsibility of making sure the lab was up to snuff. I would change the legal twist to say it is OK if you profit, but the service must be as good if not better than the alternatives. There should also be no evidence that the services were ordered when not needed. With these provisions, let the doctor own the laboratory.

In the case of this CNN story, it is unclear as to whether the quality of the care suffered because of these supposed relationships.

Face it people: if we dod not allow a pathway for those that have endured years of education to prosper at the end of this training, we will have no more quality medicine in this country. We are headed that way. Do you really want to arrive?

P.S. At this time I have no such relationships, but think it is silly that these kinds of restrictions exist in healthcare and few if any other industries.

A Model and her Breasts - From the Forum

A question from the forum shows that thin women can have a hard time getting natural-looking breast augmentation results if they do not choose small implants and/or get them placed below the pectoral muscles:

She's Giorgia Palmas, a very popular italian soubrette. I think those things are quite fake looking, but maybe I'm paranoid or something. Any professional comment (if are fake, do you think it was possible to make "them" less fake looking? Wink). And the girl is just barely 20 years old, by the way.


An Answer:

Her breasts look implanted to me. She probably went too large and/or had her implants placed "over the muscle." Models frequently have little body fat and want relatively large breasts. Ladies who are slender look more natural with smaller breast implants. Everything is relative.
